Survey to be completed by midday Tuesday

In order to generate statistics to counter claims that hospitality is a major environment for COVID transmission, we kindly ask that you answer this very short survey as soon as possible. We hope to have enough responses to be able to include findings in briefings to MPs, prior to an important series of votes in Parliament tomorrow evening, so your swiftest response would be greatly appreciated.
